The North Star Guide is a practical tool for exploring your values, beliefs, and wishes around end-of-life care. Through guided reflection and conversation, you will consider what matters most to you, translate those insights into tangible plans, and create a personalized set of documents for advance care, legacy, and memorial preferences. While these plans are not legal documents, they can be revisited and updated over time, serving as a living record of your priorities, intentions, and legacy.

More than just planning for death, this process encourages reflection on life, helping you live with intention, purpose, and a deeper sense of connection. Whether preparing for yourself or supporting a loved one, the North Star Guide provides clarity, confidence, and a framework for meaningful conversations and decisions.

The process begins with a Values Worksheet, followed by sessions structured around four key areas: Physical, Emotional, Spiritual, and Practical. A reflection and review session midway through ensures that the work remains aligned with your goals, vision, and evolving needs.

This work is deeply personal and flexible, incorporating your unique values, ethics, and morals. It encourages reflection on life, legacy, and relationships, helping you live with intention, purpose, and connection. You will have the opportunity to explore questions that may not have been addressed before, including decisions about hospice, advance care, memorial preferences, and personal reflections.

three-month follow-up session is included as part of the package, with additional follow-ups at six months to continue refining plans and addressing emerging questions. Beyond these, any additional sessions can be arranged as a separate package.

While the process can provide clarity and emotional preparedness, it is not a guarantee—your engagement and reflection are essential to creating meaningful results.
